Gist: Cook's frame is still the cleanest antidote to simple root-cause stories. Complex systems are intrinsically hazardous, but they are also heavily defended, so major failures usually require several small, individually insufficient faults to line up. The system may look normal because it runs in degraded mode all the time, with people constantly adapting around latent problems.
That makes post-accident blame misleading. Hindsight makes one operator action or technical fault look decisive, but at the sharp end people are balancing production pressure, ambiguity, and incomplete information. Safety is not a property bought once or located in a single component; it is continuously recreated by practitioners who understand where the edge of the envelope is.
